Bridge Care

The Bridge Care Ulm is a service of the University Hospital Ulm and is aimed at patients with an incurable, advanced illness who have an increased need for care and would like to spend as much time as possible at home.

At home in difficult times - The Bridge Care provides support for patients with incurable tumor diseases and helps them to spend the remaining time at home.

Our goal - to give those affected and their relatives security and to enable them to cope with challenging situations in the home environment.

Our services include

Organization of home-based palliative care: We coordinate nursing services, medical supply providers, and other service providers.
Complex care transitions: We ensure that all necessary measures are implemented seamlessly, also in cooperation with specialized outpatient palliative care (SAPV).
Advance planning: Together with you, we develop strategies to manage crisis situations at home—e.g., pain management or drainage care.
Instruction and support for family members: We train your family members in caregiving situations and provide them with advice and support.
24-hour on-call service: We are available for you at any time in emergencies.
Coordination of medical care: We work closely with your primary care physicians and hospital doctors.

Requirements for supervision
Costs

The following is required so that we can provide you with the best possible support:

You are being treated at Ulm University Hospital.
You are at an advanced tumor stage with no prospect of a cure.
You live within a radius of 20 kilometres.

There are no costs for your care.

Bridge Care is financed by the hospital and donations.

In the case of care provided by SAPV, the respective health insurance fund will cover the costs after checking your entitlement.

Team

The team consists of nursing staff with further training in palliative care and many years of experience in the care and support of seriously ill and dying patients.
